Controlled Governance Procedure

1. Purpose

This procedure defines the controlled steps used by {{company_name}} to execute, evidence, approve, and review a governed process.

2. Scope

The procedure applies whenever {{trigger_event}} occurs or an authorised operator initiates the governed workflow.

3. Roles and Responsibilities

4. Preconditions and Inputs

Before execution, the responsible role SHALL confirm that required inputs, authorisations, source records, and applicable instructions are available and current.

5. Procedure Steps

  1. Record the trigger and create the procedure instance.
  2. Gather and validate required inputs.
  3. Execute the prescribed control or operational actions.
  4. Record decisions, exceptions, and supporting evidence.
  5. Escalate unresolved or material exceptions.
  6. Submit the completed instance to {{approver_role}} where approval is required.
  7. Store final evidence in {{evidence_repository}}.

6. Decision Points and Exceptions

Decision points SHALL identify the condition evaluated, permitted outcomes, responsible decision-maker, evidence used, and resulting path. Exceptions SHALL be documented with rationale and approval.

7. Evidence and Outputs

Execution records, approvals, exceptions, and supporting artefacts SHALL be retained in {{evidence_repository}} with sufficient provenance to reproduce the completed workflow.

8. Escalation and Review

Material failures, repeated exceptions, or control weaknesses SHALL be escalated to {{procedure_owner}} and reviewed for corrective action.
