Controlled Governance Procedure
1. Purpose

6. Decision Points and Exceptions
Decision points SHALL identify the condition evaluated, permitted outcomes, responsible decision-maker, evidence used, and resulting path. Exceptions SHALL be documented with rationale and approval.

Execution records, approvals, exceptions, and supporting artefacts SHALL be retained in {{evidence_repository}} with sufficient provenance to reproduce the completed workflow.
8. Escalation and Review
